Basalt caecilians are huge worm-like creatures that look like two-headed long snakes with a very thick hide, about 4 to 5 feet in diameter.[4] They have prominent mandibles that, when open, reveal cylindrical rows of teeth,[5] They have a stinger at the end of their tails.[6]

Bells Hells encountered a basalt caecilian in a cave or tunnel in the cliffs along the shore of Kalutha.[7]
Abilities[]
- Multiattack: The creature can use its Extend Stomach attack, plus make two bite attacks.
- Armor class: 17[11]
- Lair actions: Pods holding larvae fall to the ground, inflicting minor acid damage[12]
- Inflicts acid damage on those in melee with it when hit[13]
- Self-healing[14]
- 10-foot range on attacks
- Tremorsense[15]
